The short answer
A curved upper back does not develop in everyone with the condition. Its appearance and effect vary, so an individual assessment matters more than comparing posture with someone else's.
Why the spine may curve
Ongoing inflammation can cause pain and stiffness around the joints of the spine. Over time, healing may produce new bone and some vertebrae may join together, reducing flexibility. If the spine becomes fixed while held forward, the upper back can look rounded and the head may sit farther in front of the body.
Muscle tightness, pain-avoiding posture, hip stiffness, and reduced chest movement can also contribute without complete spinal fusion. A clinician can examine whether the curve remains flexible and whether imaging is needed to assess structural change.

Keep rheumatology follow-up appointments and discuss persistent stiffness or a noticeable posture change. Treatment may include medication to control inflammation alongside physiotherapy focused on spinal mobility, hip movement, chest expansion, strength, balance, and safe posture habits.
Use an exercise plan chosen for your current mobility rather than forcing the spine straight. Break up long periods of sitting, arrange screens so you are not continually bending forward, and stop any movement that causes sharp pain, new tingling, or weakness.

Arrange a clinical review if your posture is changing, pain is disrupting sleep, or daily tasks are becoming harder. Seek urgent assessment after a fall or sudden spinal pain, particularly if your spine is already stiff, because a rigid spine can be vulnerable to injury.
