How location and movement provide clues

Pain along the inner ankle toward the arch can occur when a supporting tendon is overloaded. Discomfort across the front of the ankle or top of the foot may relate to joint irritation, tight footwear or tendons that lift the foot. Pain after a twist may accompany an ankle sprain, while direct bone tenderness after trauma calls for fracture assessment.

Morning stiffness, pain during push-off, symptoms on uneven ground and a change in the arch shape are useful observations. Burning, tingling or numbness suggests that nerve irritation should also be considered.

Ways to reduce irritation

Temporarily scale back hills, running, jumping and long walks that provoke symptoms. Check that shoes do not press across the painful area and that they hold the heel securely. Gentle ankle circles or up-and-down movement are suitable only within a comfortable range.

If symptoms are linked to training, note changes in distance, surface, footwear and recovery. Persistent pain may benefit from an examination of foot posture, tendon strength, balance and walking mechanics; sports physiotherapy may then provide a graded exercise plan.

When prompt assessment is needed

Seek urgent care after injury if you cannot take steps, the foot or ankle is misshapen, a bone is acutely tender, or swelling and pain are rapidly increasing. A cold, pale or blue foot, loss of sensation, significant weakness or an open wound needs immediate attention.

Arrange a routine assessment for pain that keeps returning, alters your gait, wakes you, or does not ease after activity changes. New redness, warmth or fever requires quicker medical advice.
