Can you have angina with a normal ECG?

An ECG records the heart's electrical activity during a brief period. It can reveal rhythm problems and some signs of current or previous heart injury, but stable angina is often triggered by increased workload. Between episodes, the tracing may not contain changes that explain exertional discomfort.

The result still contributes useful information. Clinicians combine it with the character and location of discomfort, its relationship to exertion, associated symptoms, examination findings, and cardiovascular risk. Depending on that assessment, imaging or testing that examines the heart under stress may be considered.

Return for clinical review if chest pressure, tightness, heaviness, or burning keeps occurring with activity, even when the ECG report says normal. Record what provokes each episode, whether discomfort spreads, any breathlessness or nausea, and how it responds to rest or prescribed medicine.

The next step is chosen from the entire picture rather than the ECG alone. Your clinician may reassess other possible causes, arrange further heart investigation, or refer you to a specialist. Continue prescribed treatment unless the prescriber advises a change, and make sure you understand the plan for recurrent symptoms.

Request emergency assistance for chest discomfort that is severe, new, occurs at rest, persists, or is accompanied by sweating, breathing difficulty, nausea, fainting, or pain spreading to the arm, jaw, neck, or back. A previous normal ECG cannot clear a new episode. Do not wait for a routine test or travel alone by car.

Ask what the tracing did show, whether the symptom history remains compatible with angina, and which alternative diagnoses are being considered. Clarify whether further testing is warranted, how current medicines should be used, and which symptom changes require emergency action.
