Why heart discomfort may be felt in the neck
Signals from the heart and nearby areas travel through overlapping nerve pathways. The brain may therefore interpret reduced blood flow to the heart muscle as discomfort in the neck, jaw, shoulder, arm, back, or upper abdomen. This is called referred pain.
Angina often presents as pressure, tightness, heaviness, or an ache rather than a sharply localized pain. A pattern brought on by walking, climbing, cold air, or strong emotion and eased by rest can fit angina. Neck discomfort can also arise from muscles, joints, nerves, teeth, or the throat, so location alone cannot identify the cause.
What to do when the neck symptom occurs
Pause exertion and note whether there is chest pressure, breathlessness, sweating, nausea, dizziness, or discomfort elsewhere. Follow the attack plan given by your clinician if you already have diagnosed angina and prescribed medicine. A new exertional pattern warrants prompt medical review even if rest makes it fade.
At the appointment, explain what you were doing at onset, how the feeling developed, where it travelled, and what relieved it. The clinician may assess the heart and circulation, perform an ECG, and decide whether additional testing is needed.
Red flags requiring urgent help
Contact emergency services now if neck or jaw discomfort is new or intense and occurs with chest pressure, breathing difficulty, cold sweating, nausea, collapse, or pain spreading through the upper body. Urgent help is also needed when a familiar angina episode happens at rest, feels different, worsens, or fails to settle according to the prescribed plan. Avoid driving yourself.
