Understanding NICE guidance on anaphylaxis

NICE is the UK's National Institute for Health and Care Excellence. Its recommendations can help explain a care pathway, but patients in the UAE should follow local emergency services, their treating clinician and their personal action plan.

The pathway begins with clinical recognition: sudden airway, breathing or circulation problems after a likely allergen exposure can indicate anaphylaxis. Treatment takes priority over diagnostic tests, and skin symptoms are not required for a serious reaction to be present.

After stabilisation, the clinical team records the circumstances and symptoms, considers timed blood sampling where appropriate, and observes for recurrence. Decisions about observation depend on the reaction, treatment response, medical history and access to further care rather than a single timetable for everyone.

During a suspected reaction, use prescribed adrenaline promptly as the personal action plan directs and call emergency services. After discharge, keep a record of the suspected trigger, symptoms and treatments, attend the recommended allergy referral, and practise using the trainer version of any prescribed auto-injector.

Check the device expiry and storage instructions, carry it as advised, and ensure close contacts know where it is and how to help. Avoid the suspected trigger according to clinical advice without attempting a home exposure test.

Call for urgent help if a possible allergic reaction causes throat or tongue swelling, a hoarse voice, difficulty breathing, wheezing, collapse, confusion, severe dizziness or rapidly worsening symptoms. Give prescribed adrenaline without waiting for hives or for an antihistamine to take effect.

Emergency assessment is needed even when symptoms improve, because further monitoring or treatment may be required. If the person becomes unresponsive and is not breathing normally, begin cardiopulmonary resuscitation if able and follow the call handler's directions.
