Recognising the pattern
A person may have throat tightness, a hoarse voice, noisy breathing, wheezing, swelling, dizziness or collapse. Hives, flushing, itching, abdominal cramps, vomiting or diarrhoea can accompany these features, but no single symptom is required in every reaction.
Features across body systems
Airway involvement can cause tongue or throat swelling, difficulty swallowing, voice change or a harsh sound while breathing in. Lower-airway involvement may present with chest tightness, persistent cough, wheezing or difficult breathing. Circulatory involvement can cause pale or clammy skin, weakness, confusion, faintness or loss of consciousness.
Skin findings are often easy to notice, yet their absence must not delay treatment when breathing or circulation is affected. In babies and young children, sudden limpness, unusual quietness, persistent coughing or repeated vomiting after a likely exposure may be important clues.
How to respond
Use the prescribed epinephrine auto-injector at once when anaphylaxis is suspected, following the person’s action plan and the device directions. Inject into the outer thigh, record when it was given, and call local emergency services. Do not substitute an antihistamine or inhaler for epinephrine.
Keep the person still and lying down if possible. Someone struggling to breathe may be more comfortable sitting with legs extended, while an unconscious breathing person should be placed on their side. Stay with them and be ready to report the trigger, symptoms and treatments given.
Warning features needing emergency help
Call emergency services for sudden breathing difficulty, throat or tongue swelling, voice change, faintness, confusion or collapse after a possible trigger. Prompt action is also needed when symptoms affect different systems together, such as widespread hives with vomiting or dizziness.
An evaluation remains necessary after improvement because another phase of symptoms can occur. Emergency staff can monitor the airway and circulation and decide whether additional medication or observation is needed.
