Anal pain during sex

Pain is a signal to pause, not something you need to push through. The location, timing and character of the pain, along with bleeding, bowel symptoms, pelvic pain or skin changes, can help a clinician identify the cause.

The anus does not produce its own lubrication, so friction or rapid penetration can irritate delicate tissue or cause a small tear. Haemorrhoids, constipation, infection, inflammation and involuntary tightening of the pelvic floor can also make contact painful. Skin soreness, itching, ulcers, discharge or a new lump point to issues that warrant examination.

Pain felt deeper in the pelvis may have a different source. Endometriosis can present with deep pain during sex, particularly when tissue behind the vagina or near the bowel is involved. Pelvic inflammatory disease can present with deep pelvic pain during sex alongside symptoms such as unusual discharge, bleeding between periods or lower abdominal pain. These presentations do not establish a diagnosis on their own.

Stop if penetration hurts. Allow more time for arousal and relaxation, communicate about pace and depth, and use plenty of a compatible lubricant. Add more lubricant as needed and avoid numbing products, which can hide pain that would otherwise warn you about tissue injury. Condoms and sex toys should be used according to their material and cleaning instructions.

Avoid penetration while soreness, a suspected tear or bleeding is healing. If constipation contributes, discuss safe stool-softening measures with a pharmacist or clinician. Arrange a medical or sexual-health appointment when pain recurs, prevents wanted sexual activity, or comes with bowel, urinary, vaginal or skin symptoms. An examination is offered with your consent, and you can ask for a chaperone or stop at any point.

Seek urgent care for heavy or continuing bleeding, severe or rapidly worsening pain, fainting, fever with pelvic or abdominal pain, pus-like discharge, or inability to pass stool or gas with abdominal swelling. Prompt assessment is also appropriate after an injury, an object becoming stuck, possible exposure to a sexually transmitted infection, or any sexual contact without consent.

Describe whether pain is at the opening or deeper, whether it begins on entry or later, and whether it continues afterwards. Ask if examination, infection testing or pelvic-floor assessment would clarify the cause; what can be used safely while tissue heals; and whether bowel or gynaecological symptoms suggest a separate evaluation.
