Alpha-gal allergy testing: results and next steps

Alpha-gal is a sugar molecule found in meat from mammals and in some other mammal-derived products. Reactions can begin several hours after eating, so the connection may be less obvious than with allergies that cause immediate symptoms.

A positive test shows sensitisation, meaning the immune system has made IgE that recognises alpha-gal. It does not prove that every exposure causes symptoms. A clinician compares the value with the foods or products involved, delay before symptoms, tick exposure and features of each episode.

A negative test may make the diagnosis less likely, but timing, test method and the wider clinical story still matter. Skin-prick tests using standard meat extracts may not answer the same question as specific IgE blood testing. Supervised food challenges are specialist procedures and should not be attempted at home.

Write down what you ate, ingredients, medicines or supplements taken, when symptoms began and how they progressed. Photographs of a rash and packaging labels can help. Tell the clinician about tick bites, outdoor exposure and reactions to gelatin, dairy products or medicines if these are relevant to your experience.

Until reviewed, follow the avoidance plan already given to you and check labels carefully. Do not deliberately eat a suspected trigger to test the diagnosis. If an adrenaline auto-injector has been prescribed, carry it, know how to use it and keep an emergency plan available to people around you.

Call emergency services for difficulty breathing, throat tightness, a hoarse voice, tongue swelling, collapse, severe dizziness or rapidly worsening symptoms affecting more than one body system. Use prescribed emergency medication according to your action plan; do not wait for a rash to appear.

Arrange a non-emergency allergy review for recurrent hives, swelling, stomach cramps, vomiting or diarrhoea that repeatedly follows mammalian meat or another suspected exposure after a delay.

Ask whether your history and antibody result fit alpha-gal syndrome, which foods and non-food products matter for you, and whether an auto-injector is indicated. Also ask how follow-up testing will be used and whether coexisting asthma changes your emergency plan.
