What to do when a bald spot is getting bigger

Alopecia areata commonly presents as a smooth patch that may expand, but an enlarging spot does not establish that diagnosis. Scale, broken hairs, inflammation, hairstyle tension, hair pulling, and scarring clues can point to other causes.

With active alopecia areata, hairs near the border may shed while the central skin remains smooth and follicle openings stay visible. New patches can also appear elsewhere. Scalp ringworm more often presents with scale, itch, inflammation, or broken hairs, although appearance varies and diagnosis requires examination.

Loss caused by repeated tension often follows the stressed hairline or attachment points. A shiny surface, loss of follicle openings, persistent burning, or redness raises concern for scarring inflammation. Because these patterns require different care, enlarging size is a reason to assess the scalp rather than choose treatment from photographs alone.

Photograph the patch from the same angle with a ruler or fixed reference beside it, without pulling hairs from the edge. Check for itch, tenderness, flaking, crusting, short broken hairs, or additional patches, and write down when each feature appeared.

Stop tight hairstyles and avoid bleach, relaxers, heavy extensions, scratching, and unprescribed steroid or antifungal mixtures until assessed. Do not share combs, hats, pillowcases, or hair tools if infection is possible. A clinician may inspect the scalp closely and sample hairs or scale when fungal disease is suspected.

Arrange a timely dermatology appointment if the patch continues to widen, further spots develop, or eyebrow, eyelash, nail, or body-hair changes appear. Children with an enlarging scaly patch should also be assessed because contagious scalp infection may need prescription treatment.

Seek prompt medical care for marked pain, swelling, pus, thick crusting, fever, rapidly spreading redness, or a soft inflamed scalp lump. Faster review is also warranted when the area becomes smooth and shiny with disappearing follicle openings, since scarring disorders can damage follicles.

Ask whether the border and scalp surface suggest alopecia areata, infection, traction, or scarring loss; whether a sample or other evaluation is needed; and how enlargement should be monitored. Clarify the treatment goal and which visible changes would mean the plan needs earlier review.
