Over-the-counter allergy medicine and the combined pill

A pharmacist can identify whether a product contains an antihistamine, decongestant, pain reliever, or a mixture, and can check it against your pill and medical history.

Single-ingredient antihistamines are easier to assess than multi-symptom cold and allergy products. Some antihistamines cause drowsiness, while decongestants may be unsuitable for people with certain heart, blood-pressure, thyroid, or eye conditions. Brand names can cover different formulations, so compare active ingredients rather than relying on the front label.

Vomiting or severe diarrhoea can reduce absorption of an oral contraceptive even when the allergy medicine itself does not interfere. Follow the missed-pill or sickness instructions supplied with your combined pill and use additional contraception if those instructions advise it.

Tell the pharmacist the exact name of your contraceptive and list prescribed medicines, supplements, asthma treatments, and any previous medicine reactions. Choose the simplest product that targets your symptoms. Avoid taking two remedies that contain the same ingredient, and do not drive or operate machinery if the label warns of drowsiness.

For ongoing nasal symptoms, ask whether a non-drowsy antihistamine, saline rinse, or suitable nasal treatment makes sense. Check the leaflet for dose instructions and stop a product if it triggers a new rash, swelling, wheeze, or marked palpitations.

Call emergency services for swelling of the tongue or throat, difficulty breathing, collapse, confusion, or rapidly worsening symptoms affecting several parts of the body. Do not rely on an over-the-counter antihistamine for a severe allergic reaction. Seek a clinical review for persistent wheeze, recurring facial swelling, or symptoms that remain uncontrolled despite appropriate use.

Ask whether your preferred allergy product is compatible with your contraceptive, whether drowsiness could affect work or driving, and what to do if vomiting or diarrhoea occurs. If you have new headaches, chest symptoms, leg swelling, or major changes in bleeding while using the combined pill, describe the timing clearly.
