Symptoms of ectopic pregnancy and when to get help

The pattern differs between people and can change as the pregnancy develops. No single symptom confirms the diagnosis, and a home pregnancy test cannot identify where a pregnancy is located.

Pain may sit low in the abdomen or pelvis, affect one side, appear suddenly or gradually, and remain constant or come and go. Vaginal bleeding may differ from a usual period, while brown watery discharge can also occur. Some people notice pain when urinating or opening their bowels, diarrhoea, nausea, or general abdominal discomfort.

Shoulder-tip pain can happen when internal bleeding irritates the diaphragm and nearby nerves. Weakness, light-headedness, fainting, pale or clammy skin, breathlessness, or a racing heartbeat may accompany significant blood loss. Ectopic pregnancy can present with these symptoms, but similar complaints have other causes and need proper assessment.

Seek prompt clinical advice if pregnancy is possible and you have new pelvic or abdominal pain, unusual vaginal bleeding, or brown discharge. Do this even if contraception was used, the home test is negative, or you do not feel strongly pregnant. Tell the clinician when symptoms began, where the pain is, whether bleeding is changing, and the date of your last period.

A clinician may use examination findings, pregnancy-hormone blood tests, and transvaginal ultrasound to work out the pregnancy location. Early findings can be inconclusive, so repeat testing may be arranged. Follow the stated return precautions and attend repeat appointments even if discomfort improves.

Get emergency help immediately for severe or rapidly worsening abdominal or pelvic pain, shoulder-tip pain, fainting, collapse, marked dizziness, confusion, heavy vaginal bleeding, breathing difficulty, or pale and clammy skin. Ask someone else to take you or call local emergency services if you feel faint. These are warning signs of possible internal bleeding, regardless of whether pregnancy has been confirmed.

Confirm whether the pregnancy has been seen inside the uterus, whether blood tests need repeating, and when another scan is planned. Ask exactly which changes mean returning sooner, whom to contact outside clinic hours, and whether pain relief is suitable while the diagnosis remains uncertain.
