How HIV-associated pneumonia may present

Pneumonia in someone with advanced HIV can present with cough, fever, breathlessness, chest discomfort, chills or marked tiredness. Some infections cause a dry cough and gradually worsening breathlessness; others bring phlegm and a more sudden illness. Symptoms alone cannot reliably identify the organism.

Assessment may include oxygen measurement, chest imaging, blood tests and a sample of sputum or fluid from the lungs. Clinicians also consider HIV treatment, immune status, preventive medicines and recent infection exposures.

What to do if pneumonia is suspected

Arrange same-day clinical assessment for a new cough with fever or breathlessness if you have HIV and know or suspect that your immune system is weakened. Bring your medicine list and recent HIV results if available. Treatment is selected for the suspected organism and severity, then adjusted when test results clarify the cause.

Take prescribed medicines exactly as directed and report vomiting, rash or other troublesome effects. Continue HIV treatment unless the treating clinician gives different instructions, because unplanned interruption can complicate care.

Warning signs needing urgent help

Call emergency services for severe difficulty breathing, blue or grey lips, new confusion, collapse, or chest pain that is intense or persistent. Seek urgent assessment if breathing is becoming harder, you cannot keep fluids or medicines down, or you feel rapidly worse.

Do not wait for a routine HIV appointment when respiratory symptoms are escalating. Urgency is based on breathing and overall illness, not on whether a particular infection seems common.
