Recovery after skin graft surgery

Keep dressings exactly as the surgical team directed and attend planned checks. The graft and the area where skin was taken are separate wounds, so each may have different dressing and activity instructions.

The transplanted skin initially relies on close contact with the wound bed, then develops a blood supply from tissue underneath. A secured dressing or bolster limits shearing and fluid collection during this delicate phase. Colour can change as circulation develops, and the surface may look unfamiliar before it matures.

The donor site heals like a separate superficial or surgical wound, depending on the graft type. It may feel raw, tight or itchy. Reduced sensation, altered colour and firmness can occur in either area, while scar appearance continues to evolve after the wounds close.

Do not remove, wet or adjust a dressing unless instructed. Position or elevate the operated area as advised, avoid stretching across the graft and follow movement or weight-bearing restrictions. Take prescribed medicines as directed and keep smoking or vaping away from recovery because nicotine narrows blood vessels.

Once the team confirms wound closure, they may introduce moisturising, scar massage, compression or sun protection in stages. Do not begin these over an open area. Later scar treatments, including fractional laser treatment, require specialist timing and are not a substitute for early wound care.

Call the surgical team if a dressing slips, becomes soaked with blood or fluid, develops an unpleasant smell, or if pain and swelling are increasing. Prompt assessment is needed for fever, spreading redness, pus, wound separation, new bleeding that does not stop with the advised pressure, or a graft that becomes markedly pale, dark or cold. Seek emergency help for breathing difficulty, chest pain or sudden severe illness.

Before leaving care, confirm which dressing can be changed, how to bathe, what movement is restricted and whom to call if the graft changes colour. Ask when work, driving and exercise can resume, how the donor site should be managed, and when scar care will be safe to start.
