What postpartum leg swelling can mean
Fluid retained during pregnancy does not disappear immediately after delivery, and intravenous fluids given around labour or a caesarean can add to puffiness. This tends to affect both feet, ankles or lower legs. A deep vein thrombosis can present with swelling mainly in one leg, often with calf pain, tenderness, warmth or skin colour change. Appearance alone cannot confirm the cause.
Comfort measures for even swelling
When swelling is mild, even on both sides and no warning signs are present, rest with the legs supported, change position regularly and take short gentle walks as recovery allows. Wear loose footwear and avoid tight bands around the calves or ankles. Drink normally; restricting water does not reliably remove tissue fluid and may leave you dehydrated.
Compare both legs and notice whether swelling is improving or extending upward. If you had a caesarean, prolonged immobility, a previous clot or were given injections to prevent clots, follow the maternity team's movement and medication instructions exactly. Do not massage a painful, hot or unusually swollen calf.
Signs that should not wait
Get urgent medical assessment for new swelling mainly in one leg, especially with calf pain, warmth, redness or tenderness. Call emergency services for sudden breathlessness, chest pain, coughing blood, collapse or a racing heartbeat. Severe headache, vision changes, pain high in the abdomen, nausea or sudden swelling of the face or hands after birth can accompany postpartum pre-eclampsia and also need urgent assessment.

Explain when the swelling began, whether one side is larger, and whether there is pain, warmth, breathlessness, headache or visual disturbance. Ask whether your blood pressure needs checking, whether your clot risk changes the urgency, and which compression garments or activity level are suitable for your type of delivery and current recovery.

Should I drink less water to reduce swollen ankles?
No. Drink according to thirst and your clinician's advice. Deliberately cutting fluids can cause dehydration and does not directly draw swelling out of the tissues.
