Planning and monitoring a pregnancy after an ectopic pregnancy

An ectopic pregnancy develops outside the main cavity of the uterus, commonly in a fallopian tube. Having had one means a later pregnancy deserves earlier assessment, although it does not determine the outcome of that pregnancy. The clinician will consider how the ectopic was managed, whether a tube was removed or retained, and whether there is a history of tubal damage or pelvic infection.

Blood tests may be used to follow the pregnancy hormone pattern. A transvaginal ultrasound can then look for a pregnancy inside the uterus when it is expected to be visible. A scan performed very early may be inconclusive, so repeat testing can be needed.

Before trying to conceive, confirm that follow-up from the ectopic pregnancy is complete. Ask when it is medically suitable to try again because recovery advice differs after surgery and medicine. Discuss folic acid, current medicines, smoking, long-term conditions and emotional readiness during pre-pregnancy care.

After a positive pregnancy test, tell the maternity team promptly about the previous ectopic and the treatment received. Arrange the blood tests or early scan they recommend, and keep the agreed follow-up even if you feel well. Write down any bleeding, one-sided discomfort, dizziness or shoulder-tip pain so changes can be described clearly.

Seek urgent medical assessment during a possible or confirmed pregnancy for severe or increasing pelvic or abdominal pain, pain focused on one side, shoulder-tip pain, heavy bleeding, fainting, marked dizziness, weakness or feeling acutely unwell. These symptoms can indicate internal bleeding and should not wait for a routine scan. Contact the maternity team promptly for lighter bleeding or persistent pelvic pain so they can decide the appropriate checks.

Ask whether follow-up from the ectopic is complete, how its treatment affects when to try, and who to contact after a positive test. You can also ask when an ultrasound is likely to answer the location question, whether pregnancy-hormone blood tests are planned, and what symptoms should take you directly to emergency care.
