Swollen legs after a C-section: expected changes and red flags

Short answer
Leg swelling after a C-section may come from retained fluid and reduced movement, while one-sided painful swelling requires urgent medical assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Compare
- Check whether one leg is more swollen than the other
- Movement
- Gentle walking and ankle motion may help when medically cleared
- Emergency symptoms
- Chest pain, sudden breathlessness, coughing blood, or collapse
Reading the pattern of swelling
Fluid retained during pregnancy and fluids given through a vein around surgery can settle in the lower legs. Reduced movement during early recovery can add to pooling. Similar swelling in both legs that slowly improves with elevation and movement often fits this pattern.
A clot in a deep leg vein can present with swelling mainly on one side, often alongside calf or thigh pain, warmth, tenderness, or a change in skin colour. Postpartum pre-eclampsia can present with raised blood pressure, severe headache, vision disturbance, upper abdominal pain, nausea, breathlessness, or sudden swelling of the face or hands. Symptoms and examination, rather than swelling alone, guide the assessment.
Safe steps during recovery
Compare both legs and notice whether the swelling changes, becomes painful, or spreads. When your surgical team has cleared you to move, take gentle walks and move the ankles while sitting or lying down. Elevate the legs during rest and avoid clothing that digs into the groin, knees, or ankles. Keep taking prescribed clot-prevention medicine for the full instructed course. Do not massage a leg with possible clot symptoms or take a diuretic unless it has been prescribed.
Red flags after caesarean birth
Get urgent medical help for new one-sided leg swelling, calf or thigh pain, warmth, redness, tenderness, or a leg that suddenly feels heavy. Call emergency services immediately for sudden shortness of breath, chest pain, coughing blood, collapse, or severe breathing difficulty. A severe headache, visual changes, pain beneath the ribs, vomiting, or sudden facial or hand swelling also warrants prompt maternity assessment.
Contact your care team if swelling is increasing rather than settling, makes walking difficult, or occurs with fever, wound discharge, worsening incision pain, or feeling generally unwell.
Details to share with your care team
Tell the clinician which leg is affected, where the swelling starts and ends, whether there is pain or warmth, and how quickly it changed. Ask whether you need a blood-pressure check, a leg ultrasound, a medication review, or adjusted guidance on walking and exercises after surgery.

Questions people ask
Is it normal for both legs to swell after a C-section?
Both legs can swell as retained pregnancy fluid and fluids used around surgery move into lower tissues. The swelling should still be monitored for worsening or additional symptoms.
Why is swelling in one leg more concerning?
A deep-vein clot can present with swelling mainly in one leg, particularly when pain, warmth, redness, or tenderness is present. This pattern needs urgent assessment.
Can I walk with swollen legs after surgery?
Gentle walking is often part of recovery once your maternity team says it is safe. Stop and obtain urgent advice if movement brings new chest symptoms or the leg has possible clot signs.
