Adenovirus pneumonia: symptoms, care and red flags

The virus can inflame the airways and air sacs, where oxygen normally enters the blood. Swelling, mucus and fluid in these spaces can produce a persistent cough, chest discomfort, fever, faster breathing and breathlessness. Children may show poor feeding or unusual sleepiness rather than clearly describe chest symptoms.

Clinical assessment may include temperature, breathing rate, oxygen level and listening to the chest. Depending on severity and uncertainty, a clinician may request a chest X-ray or samples from the nose, throat, blood or sputum. Symptoms overlap with influenza, COVID-19 and bacterial pneumonia, so examination and testing guide management.

Rest, take regular fluids and avoid smoking or vaping. Fever or pain medicine may improve comfort when suitable for your age, health conditions and other medicines. Follow infection-control advice, including handwashing, covering coughs and avoiding close contact with people at higher risk while acutely unwell.

Antibiotics do not treat adenovirus, though they may be needed if clinicians find a bacterial infection as well. Hospital care can include oxygen, intravenous fluids and closer monitoring. People with reduced immunity may require specialist advice because illness can be more difficult to control.

Call emergency services for severe difficulty breathing, blue or grey lips, collapse, new confusion, inability to wake normally, or intense chest pain. A child needs emergency attention for pauses in breathing, grunting, marked pulling in beneath the ribs, or being too breathless to feed.

Seek prompt clinical review for worsening breathlessness, reduced urination, inability to keep fluids down, coughing blood, fever with increasing weakness, or improvement followed by a clear deterioration. Older adults, pregnant people, young children and anyone with heart, lung or immune problems should have a lower threshold for assessment.

Ask whether oxygen measurement or imaging is needed, how to distinguish expected recovery from deterioration, and when activity can gradually resume. Clarify whether any regular medicine should change during the illness and whether follow-up is advised for a persistent cough or breathlessness.
