Adalimumab for psoriatic arthritis: what patients should know

It is a biologic medicine that blocks tumour necrosis factor, an immune-system signal involved in inflammation. A clinician may consider it when psoriatic arthritis remains active or when other treatment is unsuitable. It is given by injection under the skin according to an individual prescription.

Psoriatic arthritis can present with joint pain, swelling and morning stiffness, alongside skin or nail psoriasis. By dampening a specific inflammatory pathway, adalimumab aims to control disease activity and protect day-to-day function. Response is reviewed over time; improvement may involve easier movement, less swelling or better control of skin symptoms.

Because the medicine changes immune activity, clinicians commonly check for tuberculosis, hepatitis and current infection before treatment. Vaccination plans, pregnancy intentions, other medicines and previous reactions to biologic drugs should also be discussed. Live vaccines may be unsuitable during therapy, so vaccination advice should come from the treating team.

Follow the prescribed injection schedule and storage instructions. Learn the injection technique from a healthcare professional, rotate injection sites, and avoid injecting into skin that is sore, bruised, infected or affected by active psoriasis unless specifically directed. Do not double a dose after a missed injection; contact the prescribing service for instructions.

Keep planned blood tests and treatment reviews, and report new infections before the next dose. Tell every clinician treating you that you use adalimumab, particularly before surgery or dental procedures. Movement and physiotherapy may still support strength and mobility, but they complement rather than replace control of inflammatory disease.

Contact the treating team promptly for fever, persistent cough, breathlessness, painful urination, an infected wound, shingles, unusual bruising, marked fatigue or yellowing of the skin or eyes. The clinician may advise delaying a dose while an infection is assessed; do not make that decision without clinical guidance unless emergency care takes priority.

Seek emergency help for trouble breathing, swelling of the face or throat, faintness, severe chest symptoms, sudden weakness, or a rapidly worsening allergic reaction after an injection. New neurological symptoms, severe vision changes or a serious widespread rash also warrant urgent assessment.

Ask what improvement would show that the medicine is working, which tests are needed before and during treatment, and whom to contact if you develop an infection or miss a dose. It is also useful to clarify vaccine timing, travel storage, injection training, and whether any current medicine increases infection risk.
