Acute Gout Flare Treatment: Choosing Safe Relief

A clinician may select a non-steroidal anti-inflammatory medicine, colchicine, or a corticosteroid. The safest option depends on factors such as kidney function, digestive or heart conditions, infection risk, pregnancy, blood-thinning treatment, and potential medicine interactions.

Flare medicines suppress the painful inflammatory response to urate crystals; they do not remove the underlying crystal burden during the episode. Daily urate-lowering treatment serves a different purpose by changing urate levels over time. Both may form part of one care plan, but their instructions are different.

Treatment choice is individual rather than a simple ranking. An oral medicine may be unsuitable because of kidney impairment, ulcer or bleeding risk, or interactions. In some circumstances, a clinician may consider a corticosteroid by mouth or injection after deciding that infection is not the cause of the swollen joint.

Begin the clinician-approved flare plan when familiar symptoms start, following the prescription exactly. Rest and elevate the joint, remove pressure from clothing or bedding, and use a cool pack wrapped in cloth. Keep to your usual fluid guidance, particularly if kidney or heart disease limits intake.

Check the ingredient labels of over-the-counter products so that the same anti-inflammatory drug is not taken twice. Unless your prescriber says otherwise, do not alter ongoing urate-lowering treatment simply because a flare has begun.

Fever, chills, profound unwellness, spreading skin redness, a nearby wound, recent joint injection or surgery, or reduced immunity can accompany joint infection, which cannot be safely managed as a routine gout flare.

Contact the prescriber promptly for black stools, vomiting blood, unusual bleeding, severe diarrhoea or vomiting, facial swelling, breathing difficulty, a widespread rash, or markedly reduced urine after taking treatment. Also seek review if pain continues to escalate or the expected response does not occur.
