The short answer

Use the flare plan previously agreed with your clinician if you have a confirmed diagnosis and the current episode matches your usual pattern. A new or unusual hot joint should not be assumed to be gout because joint infection can cause a similar presentation.

What is happening inside the joint

Crystals formed from urate can collect around a joint and activate the immune system. The resulting inflammation makes even light pressure painful and can sharply restrict walking or hand use. Attacks may involve the toe, ankle, knee, wrist, fingers, or elbow and do not have to recur in the same place.

An attack describes the inflammatory episode; it does not show the full pattern of urate control between episodes. Symptoms can settle while crystals remain. That is why follow-up prevention and the immediate pain plan are related but separate parts of care.

Actions to take when an attack starts

Reduce weight and pressure on the affected joint, elevate it, and apply a covered cool pack if soothing. Maintain your usual sensible fluid intake unless a clinician has restricted fluids. Record when symptoms began and any recent illness, dehydration, medicine change, or unusual intake that could help explain the trigger.

If you have an agreed prescription for attacks, follow its dosing instructions promptly. Do not combine anti-inflammatory products or borrow another person's medicine. Contact a clinician or pharmacist before treatment if your health has changed, particularly if you now have kidney problems, an ulcer, bleeding risk, pregnancy, or new prescribed medicines.

Red flags and unexpected patterns

Arrange urgent evaluation for your first hot and swollen joint, severe symptoms without a confirmed gout diagnosis, or an attack that differs from your established pattern. Fever, shaking chills, feeling systemically unwell, spreading redness, broken skin near the joint, or a recent procedure on that joint increases concern for infection.

Get reviewed if the prescribed plan is not controlling symptoms, side effects develop, or several joints become inflamed. A clinician can reconsider the diagnosis, check for treatment complications, and decide whether joint-fluid sampling or other tests are needed.

Build a plan for the next attack

Ask which medicine to begin at the earliest familiar symptoms, what side effects mean you should stop and seek advice, and how other prescriptions affect the choice. Clarify what to do with your daily urate-lowering treatment and which changes in attack frequency or pattern should prompt a prevention review.
