What the image can mean

Acute chest syndrome is a lung complication associated with sickle cell disease. On an X-ray, clinicians look for a new infiltrate or opacity, which means part of the lung appears whiter than expected. This may reflect infection, inflammation, blocked small blood vessels or areas of lung that are not fully expanding.

The appearance can overlap with pneumonia, and the X-ray alone cannot identify the exact cause. A normal-looking first image does not always settle the question when symptoms and oxygen readings remain concerning.

What happens next

Suspected acute chest syndrome needs prompt hospital assessment. The care team may monitor oxygen, repeat examinations, request blood tests and begin treatment before every result is available. Treatment can include oxygen, pain relief that still allows deep breathing, antibiotics, fluids given carefully and blood transfusion when clinically indicated.

Bring details of sickle cell medicines, recent pain episodes, fever, cough and any previous acute chest syndrome. Do not delay assessment to arrange an outpatient X-ray if new breathing difficulty or chest pain is present.

When to seek emergency care

Go to an emergency department now for shortness of breath, chest pain, fast or laboured breathing, blue or grey lips, confusion, unusual sleepiness, fainting or a low oxygen reading. Fever, cough or wheeze in someone with sickle cell disease also warrants urgent clinical advice because lung illness can progress quickly.

Call emergency services if the person is struggling to speak, becoming difficult to wake or cannot breathe comfortably while resting.
