Acupuncture for gout: role, limits and safety

Gout is driven by urate crystal deposits within and around joints. Controlling inflammation during a flare and managing urate over time require a medical plan; acupuncture should not replace prescribed medicine.

Gout can present with rapid onset of intense joint pain, swelling, warmth and skin redness, often making light touch difficult. Infection inside a joint can produce a similar picture and needs urgent treatment, so a presumed first flare should be assessed rather than treated with needles alone.

Kidney function, current medicines and other health conditions can influence which gout treatments are suitable. A clinician may also need to distinguish gout from injury or another inflammatory arthritis.

Rest the painful joint, protect it from pressure and follow the flare plan provided by your clinician. Take prescribed treatment exactly as directed and do not stop long-term urate-lowering medicine during a flare unless the prescriber tells you to.

Avoid needling directly through red, swollen, damaged or infected-looking skin. If you pursue acupuncture after medical assessment, tell the practitioner about medicines that affect bleeding, kidney disease, reduced sensation and any tendency toward poor healing.

Seek urgent assessment for a hot, swollen joint with fever, chills, feeling very unwell, an open wound or rapidly spreading redness. Also obtain care when this is the first episode, pain follows an injury, you cannot use the limb, or symptoms are worsening despite the agreed flare treatment.

Ask how the diagnosis will be confirmed, which medicine is appropriate with your kidney function and other prescriptions, whether urate-lowering treatment needs adjustment, and which changes should trigger an urgent reassessment.
