Understanding an acral melanoma pathology report

Short answer
An acral melanoma pathology report confirms the diagnosis and describes features that help the clinical team plan further care.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Report author
- A pathologist examining tissue under a microscope
- Important features
- Depth, ulceration and specimen margins
- Clinical use
- Diagnosis, staging decisions and treatment planning
The short answer
The report should be interpreted alongside the biopsy method, examination findings and imaging when relevant. A single technical term rarely explains the entire treatment plan.
Key terms in the tissue report
The pathologist identifies the melanoma type and measures how deeply invasive cells extend into the skin. The report may describe ulceration, cell division, involvement of nerves or vessels, and whether melanoma is present at the specimen edges. For a nail lesion, it may also distinguish changes confined to the surface layer from invasive disease.
A clear edge means tumour was not seen at the cut boundary of that specimen; it does not by itself determine whether a wider operation is advised. An involved edge means melanoma reaches a boundary and additional tissue may need removal.
Using the result to plan care
Arrange a results discussion with the dermatologist or surgical team and request a plain-language explanation of the final diagnosis, depth, ulceration and margins. If the sample was partial, ask whether the findings could change after complete removal. Further excision is commonly planned around the biopsy area when invasive melanoma is confirmed.
The team may discuss a sentinel lymph node biopsy when the tumour features make nodal staging useful. Additional scans or drug treatment depend on the full stage and individual circumstances, rather than the pathology label alone.
When to contact the care team
Contact the clinic if you have not received a clear plan after being told melanoma is present, if the result says an edge is involved, or if different reports appear to conflict. Clarifying these points helps prevent an unintended gap before definitive treatment.
After biopsy, seek prompt wound advice for spreading redness, increasing warmth, pus, fever, uncontrolled bleeding or pain that is worsening. These signs concern the healing site and should be assessed independently of the melanoma findings.
Questions that make the report clearer
Ask whether the melanoma is confined to the upper skin layer or invasive, which measurement determines its depth, whether ulceration is present, and what the margins show. Also ask whether the specimen was large enough for a firm interpretation, whether specialist review is needed, and how the result affects surgery, lymph-node assessment and follow-up.

Questions people ask
What does an involved margin mean?
It means melanoma cells reach a cut edge of the submitted tissue. The clinical team will consider additional removal based on the biopsy and overall findings.
Can a partial biopsy give the final depth?
Not always. If the deepest part was not captured, complete removal may provide a different measurement. Your clinician can explain whether the reported depth is definitive.
Why might a pathology review be requested?
A second pathologist with relevant expertise may review unusual or difficult features before the team commits to a treatment plan.
