The short answer

Reflux usually involves effortless backflow, also called regurgitation. Throwing up involves stronger abdominal contractions and may have another cause, especially when it keeps happening.

Reflux or vomiting?

With regurgitation, stomach contents rise into the throat or mouth without retching. You may notice a sour taste, burning behind the breastbone, belching, cough, or symptoms after eating or lying down.

Vomiting is a forceful emptying of the stomach and may come with nausea, sweating, or repeated retching. Infection, migraine, medicines, pregnancy, and digestive problems can also cause it, so vomiting should not automatically be attributed to reflux.

Steps you can take

After an episode, sit upright and take small, frequent sips of water or an oral rehydration drink if you can keep fluids down. Avoid lying flat, large meals, alcohol, and foods that reliably worsen your burning or regurgitation.

Keep a brief record of meals, timing, nausea, retching, pain, and any medicines used. A pharmacist or clinician can advise whether an antacid or another reflux treatment suits you, particularly if you take other medicines or have a long-term condition.

When to seek medical care

Get urgent help for vomit containing blood or material that resembles coffee grounds, green vomit, severe or worsening abdominal pain, fainting, confusion, marked weakness, or chest pressure with breathlessness, sweating, or pain spreading to the arm, back, neck, or jaw.

Arrange prompt assessment if vomiting recurs, you cannot keep fluids down, swallowing becomes difficult, urine becomes very dark or infrequent, or you are losing weight without trying. Babies, older adults, and pregnant people may need earlier advice because dehydration can develop more readily.
