How newborn reflux is managed

Bringing up small amounts of milk can happen because the valve above a newborn's stomach is still developing. Treatment depends on feeding comfort, growth, breathing, and whether vomiting has concerning features. Acid-suppressing medicine is not a routine answer for every baby who spits up.

Spitting up versus concerning vomiting

Simple reflux is often a gentle, milky spill during or after a feed while the baby otherwise feeds, settles, and produces wet nappies. Forceful vomiting, green fluid, blood, a swollen abdomen, marked pain, poor feeding, or reduced alertness does not fit uncomplicated reflux and needs medical assessment.

Crying and back arching have several possible causes, so they do not prove that acid is injuring the food pipe. A clinician may watch a feed, review volumes and technique, check growth, and consider milk-protein allergy or another illness before recommending treatment.

Safer steps at home

Feed responsively and pause for burping when the baby needs it. Keeping the baby upright against your chest for a short period after feeding may reduce spilling, but the adult must remain awake and support the head and airway. A midwife, health visitor, paediatrician, or feeding specialist can check latch, bottle teat flow, and whether feed volumes suit the baby.

For every sleep, place the baby flat on their back on a firm, level sleep surface. Do not raise the cot, use wedges or positioners, or put the baby to sleep on the front or side to treat reflux. Never add cereal or thickener to feeds unless a qualified clinician has advised the exact plan.

Medicines require paediatric assessment because dose and choice depend on the baby's weight, age, symptoms, and suspected cause. Do not give adult antacids, leftover prescriptions, herbal remedies, or over-the-counter reflux products to a newborn.

When a newborn needs medical help

Seek emergency help if the baby has trouble breathing, turns blue or unusually pale, becomes floppy or difficult to wake, has a seizure, or stops breathing. Green vomit, blood in vomit or stool, a tense swollen abdomen, or forceful repeated vomiting also needs urgent assessment.

Contact a clinician promptly for refusal of feeds, fewer wet nappies, fever, persistent coughing or choking during feeds, poor weight gain, or distress that is not settling. Newborns can become unwell quickly, so a caregiver's concern about a clear change in behaviour is a valid reason to seek advice.

Preparing for the baby's review

Bring a feeding and nappy record if practical, and describe the colour, force, amount, and timing of vomit. Ask whether a feed observation is needed, whether growth is on track, what sleep position to use, and exactly how any recommended thickener or medicine should be prepared and given.

No. Cot elevation and sleep positioners can create safety hazards. Use a firm, flat sleep surface and place the baby on their back.
