The short answer

Back or side sleeping can both work. The useful adjustment is often where the heel rests: pressure from the mattress, tight bedding or one ankle lying on the other can irritate a sensitive area.

Why the tendon may bother you at night

The Achilles joins the calf to the heel and can be tender either along the tendon or at its heel attachment. Night discomfort may come from compression against the bed, the weight of covers, or stiffness after the ankle stays still. Sleeping position does not by itself explain why the tendon became painful.

Notice whether discomfort is at the back of the heel, higher in the tendon or mainly in the calf. Also note whether it wakes you, appears only when the heel touches the mattress, or is strongest on the first steps in the morning. These details help distinguish pressure sensitivity from a broader symptom pattern.

Set up a more comfortable night

When lying on your back, place a pillow under the lower calf so the sore heel is lightly unloaded, taking care that the knee and ankle still feel relaxed. When lying on your side, use a pillow between the legs to stop the tender ankle rubbing against the other foot. Loosen tucked-in covers if they push the toes downward or press on the heel.

Do not improvise a rigid splint or force the ankle into a stretch overnight. If a clinician has prescribed a boot, brace or night device, follow its specific wearing instructions rather than general positioning advice. Daytime load management and progressive calf rehabilitation address tendon capacity more directly than sleep posture alone.

Symptoms that should not wait

Seek urgent assessment for a sudden snap, marked bruising or swelling, new inability to push off the foot, or severe pain after an injury. A hot, rapidly reddening area with fever also needs prompt medical attention.

Book a non-urgent evaluation if night pain persists, regularly interrupts sleep, worsens without a clear reason, or comes with ongoing calf or ankle swelling. A clinician can examine the tendon and review footwear, activity load and other possible sources of heel pain.

What to clarify at your visit

Ask whether the tender spot is in the tendon or at its heel attachment, whether any nighttime device is appropriate, and which daytime exercises should accompany comfort measures. If you already use a brace or boot, confirm its sleeping instructions and check that it is not causing pressure, numbness or skin irritation.
