The short answer
Its full name is anterior cervical discectomy and fusion: anterior describes the approach, cervical identifies the neck, discectomy means removing disc tissue, and fusion means encouraging adjacent bones to unite.
Structures involved in ACDF
The cervical vertebrae form the bony column of the neck. A disc between neighboring vertebral bodies provides spacing and helps movement. Behind each disc lies the spinal canal, which contains the spinal cord; nerve roots leave through side openings and travel toward the shoulder, arm, and hand.
During the anterior approach, the surgical pathway passes between soft-tissue structures rather than through the back muscles. The windpipe and food pipe are gently moved aside to reach the front of the spine. Disc material and, when necessary, bone spurs or ligament are removed to create room around compressed neural tissue.
A spacer or graft occupies the cleared disc space. A cage, plate, and screws may help preserve alignment and stability while bone bridges the treated level.
Use the anatomy to understand your plan
Ask the surgeon to point out the affected disc, nerve root, and spinal cord on your images. Confirm whether compression is central, toward one side, or present at more than one level, because this helps explain the proposed extent of decompression.
Report whether symptoms affect the shoulder, arm, particular fingers, grip, balance, or fine hand tasks. The physical examination and symptom distribution are considered alongside scans. Mention prior throat, voice, thyroid, or neck surgery because it may influence planning for the front-of-neck approach.
Symptoms that need prompt assessment
Before surgery, seek urgent assessment for newly developing limb weakness, marked loss of coordination, worsening balance, numbness around the groin, or loss of bladder or bowel control. These are neurological red flags regardless of their cause.
After ACDF, emergency help is needed for breathing difficulty or rapidly expanding swelling at the front of the neck. Notify the surgical service promptly about worsening swallowing, a draining or increasingly inflamed wound, fever, or a clear decline in strength or walking.
Questions to clarify the anatomy
Useful questions include which nerve or cord pathway is affected, what tissue will be removed, how the chosen implant fits your disc space, and whether the operation approaches from the right or left. Also ask which symptom changes would indicate expected recovery and which should trigger a call.

Why does a neck nerve problem cause arm symptoms?
