Do not assume every episode is a familiar crisis. The location, pattern, associated symptoms, and response to the person’s pain plan help the clinical team decide what needs checking.
What the pain can mean
People with sickle cell disease can develop abdominal pain when sickled red blood cells obstruct small blood vessels and reduce oxygen delivery to tissue. This vaso-occlusive pain may occur with pain in the back, chest, arms, legs, or joints, although each episode can feel different.
Other possibilities include gallstones, inflammation or infection of the gallbladder, trapping of blood in the spleen or liver, constipation related to opioid pain medicines, a urinary infection, or pancreatitis. Appendicitis and other causes unrelated to sickle cell disease remain possible. Pain under the right ribs with yellow eyes may point toward a gallbladder or liver problem; pain on the left with a swollen abdomen can involve the spleen. These clues guide assessment but do not establish a diagnosis by themselves.
What to do when pain starts
Follow the individual sickle cell pain plan, including only the medicines and doses already recommended for that person. Rest in a comfortable, warm setting and drink fluids if able to do so without vomiting or a fluid restriction. Avoid adding an anti-inflammatory medicine unless the treating clinician has confirmed it is suitable, particularly when kidney, stomach, or bleeding problems are present.
Note when the pain began, where it is, whether the abdomen looks swollen, and whether there is fever, vomiting, diarrhoea, constipation, yellowing of the eyes, dark urine, chest symptoms, or urinary discomfort. Contact the sickle cell team promptly if the pain differs from previous episodes, keeps worsening, or does not improve with the home plan. Take the current medicine list and pain plan when attending a clinic or emergency department.
When to seek urgent care
Seek urgent medical assessment for severe or rapidly worsening abdominal pain, sudden abdominal swelling, a hard or very tender abdomen, repeated vomiting, inability to keep fluids down, blood in vomit or stool, black stool, fainting, marked drowsiness, or very pale lips or skin. Fever should be assessed promptly according to the person’s sickle cell emergency plan because infection can become serious quickly.
Call local emergency services for difficulty breathing, chest pain, collapse, confusion, a seizure, or sudden weakness. Tell the emergency team immediately that the person has sickle cell disease; pain relief and evaluation for complications can proceed together.
Questions for your clinician
Ask how to distinguish this person’s familiar crisis pattern from pain that needs immediate examination, which medicines belong in the home plan, and whom to contact outside clinic hours. If episodes recur, ask whether blood testing or imaging is needed to look for anaemia, infection, gallstones, spleen changes, liver problems, or another abdominal cause.
