Abdominal pain during sex in men: what should you know?

Location and timing give useful clues. Pain linked to a particular movement or position may reflect an abdominal, hip, groin, or pelvic-floor muscle strain. Lower abdominal or pelvic pain during or after ejaculation can occur with prostate inflammation or chronic pelvic pain. Burning urine, urgency, discharge, fever, or genital discomfort can point towards a urinary or sexually transmitted infection. A groin bulge, bowel symptoms, or pain with receptive anal sex may suggest a hernia, bowel or rectal irritation, or pelvic-floor tension.

The symptom alone cannot identify the cause. A clinician may ask about erections and ejaculation, examine the abdomen and genitals, and consider urine or infection testing according to the accompanying symptoms.

Stop if an activity causes pain; pushing through it can worsen muscle guarding or irritation. Until the cause is clearer, choose positions that reduce abdominal effort and pressure, move slowly, and avoid the movement that reliably triggers symptoms. If anal penetration is involved, generous compatible lubricant and clear communication may reduce friction, but lubricant will not treat pain coming from deeper tissues or organs.

Record where the pain starts, whether it occurs with penetration, movement, erection, orgasm, or ejaculation, and how long it lasts afterward. Note urinary changes, bowel symptoms, discharge, a groin lump, recent injury, and any new sexual exposure. Arrange a routine appointment if pain returns, persists, disrupts sex, or comes with urinary or ejaculation symptoms.

Seek urgent medical care for sudden severe abdominal or testicular pain, especially with nausea, vomiting, testicular swelling, or a testicle sitting differently. Also get urgent help for inability to pass urine, severe or worsening abdominal pain, fainting, heavy rectal bleeding, or pain after a significant injury.

Prompt assessment is also needed when pelvic or abdominal pain occurs with fever, chills, blood in urine, marked pain when urinating, or rapidly increasing swelling. These features may accompany infection, urinary blockage, or another problem that should not wait for a routine visit.

Ask which structures could produce pain in the exact location you feel it, whether your urinary or ejaculation symptoms suggest prostate or urinary-tract assessment, and whether infection testing is appropriate. You can also ask if the examination suggests a hernia, muscle injury, or pelvic-floor overactivity, and what activity changes are sensible while symptoms settle.
