Bleeding with a miscarriage at six weeks

Bleeding in early pregnancy has several possible causes. A clinician may use your symptoms, examination, ultrasound and pregnancy-hormone blood tests to determine whether the pregnancy is developing, a miscarriage is occurring, or another urgent cause needs attention.

During an early miscarriage, bleeding may become heavier than a period and may include clots or pregnancy tissue. Cramping can come in waves as the womb contracts. The amount and appearance vary, so neither light nor heavy flow by itself shows whether all tissue has passed.

One-sided pain, shoulder-tip pain, dizziness or collapse raises concern for internal bleeding from a pregnancy outside the womb. That situation can occur even if vaginal bleeding seems light and requires urgent assessment.

Contact your early-pregnancy or maternity service for advice and follow the assessment plan they provide. Use pads to monitor flow, keep fluids nearby, and take only pain relief that a clinician or pharmacist has confirmed is suitable for you. If possible, have someone available while bleeding is increasing.

Follow-up may include an ultrasound, pregnancy testing or symptom review to confirm that the womb is empty and hormone levels are resolving. If tissue remains, the options may include more time, medicine or a surgical procedure, depending on your condition and preferences.

Seek urgent care if bleeding soaks through pads rapidly, pain is severe or not controlled, or you feel faint, confused, breathless or profoundly weak. Emergency assessment is also needed for one-sided abdominal pain, shoulder-tip pain or collapse.

Contact the care team promptly for fever, chills, worsening pelvic tenderness, offensive discharge, bleeding that remains heavy, or symptoms that intensify after previously settling. These can signal infection, retained pregnancy tissue or another complication that needs treatment.

Ask how to judge whether bleeding is too heavy, which pain relief is appropriate, and how completion will be confirmed. Clarify whom to call overnight, whether your blood group affects treatment, and when to repeat a pregnancy test.
