Dandruff-like flakes in a 2-month-old

It commonly appears as greasy or dry scale on the scalp and may extend to the eyebrows or behind the ears. A baby with uncomplicated cradle cap generally feeds and behaves normally and is not intensely bothered by the scalp.

Cradle-cap scale can be white, yellowish, or skin-coloured, with mild redness underneath. Eczema is more likely to be very itchy, dry, and present on other areas, while infection may cause tenderness, warmth, spreading redness, fluid, or pus. Ring-shaped patches or broken hairs need examination because they do not fit the usual pattern.

A clinician distinguishes these conditions by looking at the scale, skin beneath it, distribution, hair, and the baby’s general health. The diagnosis guides whether any medicated treatment is appropriate.

Wash gently with a mild fragrance-free baby shampoo, rinse well, and use a soft brush only on loose flakes. For firmly attached scale, a clinician may advise a plain emollient to soften it before washing. Keep oils away from the baby’s face and hands, and never force scale off.

Do not use adult anti-dandruff shampoo, essential oils, antifungal products, steroid creams, or home remedies on a 2-month-old unless a clinician has selected the product and given precise instructions. Stop any new product that causes redness or swelling.

Arrange review if the eruption spreads, becomes very itchy, cracks, bleeds, affects feeding or sleep, or fails to improve with gentle care. Prompt assessment is needed for warmth, increasing redness, swelling, pain, pus, weeping, an unpleasant smell, or a baby who is feeding poorly or unusually sleepy. A fever in a baby this young needs urgent medical advice.

Ask whether the flakes fit cradle cap or another rash, what cleanser and emollient are suitable at this age, and exactly how often to use them. Point out changes beyond the scalp and mention scratching, reduced feeding, sleepiness, fever, hair breakage, or products already tried.
