The short answer

Wheezing is usually a musical sound from the chest, often heard while breathing out. Newborn nasal congestion can make a different, snuffling noise, but it may be hard to locate the sound at home. A clinician can listen to the lungs and assess breathing effort, oxygenation and feeding.

Why a young baby may sound wheezy

Bronchiolitis can present with wheezing, cough, rapid breathing and difficulty feeding in babies. Other airway problems, infection or a sound transmitted from a blocked nose may resemble wheeze. A single sound cannot determine which explanation applies.

Record when the noise started, whether it is constant, and whether there is cough, fever, choking, vomiting or exposure to smoke. If practical, a short recording made while the baby is calm can help the clinician understand an intermittent sound without delaying care.

What to do while arranging assessment

Keep the baby away from tobacco smoke, vaping, incense and strong fragrances. Offer usual feeds only if the baby can coordinate sucking and breathing comfortably, and note wet nappies. Keep the baby upright while awake and supervised, then use a flat, firm sleep surface on their back.

Do not use someone else's inhaler, cough medicine, steam or a home nebuliser unless a clinician has examined the baby and provided exact instructions. Treatment depends on the source of the sound and the baby's breathing effort.

Emergency warning signs

Call emergency services if there are pauses in breathing, blue or grey lips, severe chest pulling, grunting, marked flaring of the nostrils, limpness, or difficulty waking the baby. Do the same if breathlessness prevents feeding.

Seek prompt medical assessment for measured fever, breathing that is persistently faster or harder than usual, fewer wet nappies, repeated vomiting, or a wheeze that is new or worsening. A baby can need assessment even without fever.

Ask whether the sound is true wheeze, what changes in feeding or breathing require immediate return, and whether any medicine is appropriate. Tell the clinician about prematurity, heart or lung conditions, birth complications, sick contacts and possible choking episodes.
