Short answer

At one month after a hair transplant, gentle routine care is usually appropriate, while shedding of transplanted hairs can be an expected part of recovery.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Common change
Temporary shedding of transplanted hair shafts
Daily priority
Gentle cleansing without picking or scratching

Worsening pain, spreading redness, pus, or fever

The short answer

The grafts should be settled, but the visible result is still immature. Follow your clinic's individual washing, grooming, exercise, and medication instructions because technique and healing history can change the plan.

What you may notice

Many transplanted hair shafts shed during the early weeks even though the follicles remain beneath the skin. This can make the recipient area look thinner before new growth becomes visible. Mild pinkness, dryness, numbness, or small spots resembling pimples may linger as the scalp settles.

Growth can be uneven, and comparing day-to-day photographs may create unnecessary worry. The donor area may also have temporary sensitivity or a tight feeling. The pattern should be judged over the longer recovery period rather than from the appearance at this single point.

How to care for your scalp

Wash with the method and products approved by your transplant team. Use fingertips rather than nails, rinse thoroughly, pat dry, and avoid picking flakes or squeezing bumps. Protect the scalp from strong sun with shade or clinic-approved headwear, and check that a hat does not rub the recipient area.

Resume strenuous exercise, swimming, hair products, colouring, or close clipping only according to your surgeon's timeline. Take prescribed medicines exactly as directed. Keep photographs under similar lighting so your follow-up discussion is based on comparable views rather than changing angles.

When to contact the clinic

Contact your transplant clinic promptly for increasing pain, warmth, spreading redness, marked swelling, pus, an unpleasant smell, fever, persistent bleeding, or rapidly worsening crusting. These changes deserve assessment because they may signal infection, inflammation, or impaired healing.

Seek urgent medical help for facial swelling that affects breathing, faintness, or signs of a severe allergic reaction. A new sharply defined bald patch outside the expected recipient pattern also warrants a clinical review, particularly if the skin is scaly, tender, or inflamed.

Useful questions for your follow-up

Ask whether your redness and shedding fit the expected course for your procedure, when you may return to your usual haircut and styling routine, which sun protection is suitable, and whether any scalp bumps need treatment. Confirm the next review date and how to send photographs if a change occurs between visits.

Questions people ask

Is shedding at one month a sign that the grafts failed?

Not necessarily. The visible shafts can shed while follicles remain in the scalp and later enter a new growth phase. Your clinic can assess unusual patchiness or inflammation.

Can I scratch an itchy transplanted area?

Avoid scratching with your nails because this can injure irritated skin. Gentle washing and a clinic-approved moisturising approach may help; persistent or severe itch should be discussed with the team.

Can I wear a hat after one month?

A loose, clean hat is often acceptable by this stage, but your surgeon's instructions take priority. It should not press, rub, or trap excessive heat over irritated skin.
