PUVA Therapy

PUVA therapy combines a light-sensitising medicine with controlled UVA exposure to treat selected skin conditions.

The medicine, called psoralen, makes the skin more responsive to UVA light. It may be taken by mouth, applied to the skin, or used in a bath before a planned light session. A clinician selects the approach and treatment area after considering the diagnosis, skin type, medicines and previous light exposure.

The light exposure itself is usually not painful. Some people notice warmth or temporary dryness afterwards. Oral psoralen can cause nausea in some people, while topical or bath methods may avoid that particular effect. Tell the treatment team promptly if you feel burning, marked tenderness or eye discomfort.

PUVA is delivered with controlled doses and monitoring, but it is not suitable for everyone. Psoralen increases sensitivity to light, so eye protection and careful avoidance of extra sunlight are important. Your clinician should review medicines, pregnancy plans, past skin cancers, photosensitivity and liver concerns before treatment.

Response and durability vary with the condition and its activity. Improvement may need a course of repeated sessions, and symptoms can return after treatment stops. Follow-up helps decide whether to pause, adjust the schedule, use maintenance treatment, or choose another option.

Treatment takes place in a clinic using a light cabinet, hand-and-foot unit, or targeted equipment. Sessions are planned rather than self-directed because both psoralen and UVA exposure affect safety. Bring an up-to-date list of tablets, supplements and skin products to each assessment.

Dermatologists may consider PUVA for selected inflammatory or pigment-related skin problems when the expected benefit outweighs light-related risks. It can be used for conditions such as psoriasis or mycosis fungoides in an appropriate clinical setting. A diagnosis should be confirmed before starting a course.

You first follow the prescribed psoralen preparation. Staff then confirm changes in your health, provide goggles and shielding where needed, and position you for the programmed UVA exposure. Afterward, the team records your reaction and tells you how to protect treated skin until its sensitivity settles.

Most people return to usual activities after a session, while following specific sunlight precautions. Wear the eye protection advised by your clinician after oral psoralen and use protective clothing outdoors. Avoid adding tanning, sunbeds or unapproved light treatments during the course.

Redness, itching, dryness and a sunburn-like reaction can occur. Longer-term cumulative UVA exposure is a reason for regular skin review. Contact the clinic if redness is intense, blisters develop, a rash spreads, or you have a reaction after taking psoralen that concerns you.

The alternatives depend on the diagnosis, distribution and severity of skin disease. Topical medicines, systemic medicines and another supervised light option may be discussed. Narrowband UVB phototherapy uses a different part of the ultraviolet spectrum and can be suitable in some treatment plans.
