Painful Breastfeeding: Causes and Relief

Pain may be on the nipple during latch, deep in the breast during feeding, or continue after the feed. It can feel pinching, burning, cracked, bruised or throbbing. A firm tender area may suggest milk is not draining freely. Nipples that look flattened, misshapen or damaged after feeding can indicate that the latch needs attention. Note whether one or both breasts are affected and whether pain is getting worse.

Painful breastfeeding can arise from nipple injury, milk not draining well or infection, and the pattern of pain helps guide assessment and treatment.

Contact a clinician promptly if breast pain comes with fever, chills, flu-like symptoms, a spreading red or hot area, marked swelling, pus, or feeling significantly unwell. Seek help for a cracked nipple that is not healing or pain that makes feeding difficult. Continuing to remove milk gently may support comfort, but urgent symptoms should not be managed by forcing vigorous massage or delaying care.

Most assessments rely on the history and examination; further tests are considered when symptoms persist or the diagnosis is unclear.

A blocked area is usually managed with comfortable feeding or expressing, rest and measures advised by a clinician; forceful deep massage can aggravate tissue. Mastitis may need medical treatment, and nipple thrush requires assessment of both parent and baby when relevant. Damaged nipples need a gentler latch and healing time. Do not stop prescribed treatment early without advice.

Bring the baby close with the nose level with the nipple, and wait for a wide-open mouth before attachment. Vary feeding positions, avoid tight pressure on the breast, and respond to fullness before it becomes uncomfortable. Keep nipples clean and dry without harsh products. Ask for help early when latch pain persists; a small adjustment can prevent worsening nipple damage or milk-flow problems.

Also mention nipple cracks, bleeding, a shiny or itchy nipple, breast redness, a lump, reduced milk flow, baby mouth changes, fever or body aches. The combination and timing of these symptoms helps distinguish feeding mechanics from inflammation or infection.

Arrange a feeding assessment if pain lasts beyond the first moments of latch, returns across feeds or affects milk removal. Early support can protect feeding confidence, identify a breast problem and help you choose safe comfort measures while the cause is treated.
