Terbinafine: an antifungal medicine for skin and nail infections

Terbinafine is an antifungal medicine, also known by the brand name Lamisil. It targets dermatophytes, the fungi commonly responsible for ringworm, athlete's foot, jock itch, and some nail infections. It may be supplied as a cream, gel, spray, or tablet, depending on the infection and the treatment plan.

Terbinafine is not an antibiotic for bacteria and does not treat every itchy or scaly rash. A clinician may examine the affected skin or nail, and sometimes test a sample, before choosing the medicine.

Terbinafine blocks an enzyme that fungi use to make ergosterol, a substance needed for a stable cell membrane. Without enough ergosterol, the fungal cell becomes damaged and may stop growing or die. Human cells use a different pathway, which helps the medicine act selectively, although side effects can still occur.

A topical product works mainly where it is placed. An oral tablet reaches the bloodstream and can deliver medicine to areas such as a nail, where creams may not penetrate sufficiently. The route, strength, and schedule should match the prescribed product.

Topical terbinafine may be used on fungal infections affecting the feet, groin, body, or other suitable areas of skin. Keep it away from the eyes, mouth, and inside the vagina unless a clinician gives different instructions. Nail infection often needs a separate assessment because a thick or discoloured nail can have non-fungal causes.

Oral terbinafine is used for selected fungal infections when a tablet is appropriate. It is commonly discussed as part of oral antifungal treatment, especially when infection involves the nail or covers an area that is difficult to treat with a topical product.

The alternative depends on the fungus, body site, extent of infection, pregnancy status, other medicines, and whether the problem is on skin, scalp, or nail. Other topical antifungals may suit a limited skin infection, while another oral antifungal may be considered for a nail or extensive infection. Antifungal treatment is not interchangeable by name alone.

Use the product for the full schedule given, even if itching or scaling settles early. Wash and dry the area gently, avoid sharing towels or footwear, and follow directions about applying the medicine beyond the visible edge of a rash. For tablets, tell the prescriber about liver disease, regular medicines, and previous reactions before starting.

Seek medical advice promptly if a tablet is followed by yellowing of the skin or eyes, unusually dark urine, marked nausea, or severe tiredness. A spreading rash, facial swelling, or breathing difficulty needs urgent help. These reactions are uncommon, but they should not be ignored.

Skin infections may improve before the skin has fully recovered, while a nail can look abnormal until healthy nail grows out. Do not extend, repeat, or share terbinafine without advice. Oral treatment may require a medical history and liver-related checks; the decision depends on the person and the planned course.
