What a hamstring autograft is

A hamstring autograft is a tendon taken from your own thigh and prepared to replace a damaged ligament during selected knee reconstruction procedures.

The tissue is usually taken from tendons along the inner or back part of the thigh. The surgeon prepares it into a graft and uses it to reconstruct a damaged ligament, commonly the anterior cruciate ligament in the knee. Because the tissue comes from you, it is not donated tissue from another person. The operation therefore has both a knee treatment site and a smaller harvest site in the thigh.

Properties of the graft

Hamstring tendons are strong, flexible connective tissues that can be folded or bundled to form a reconstruction graft. The surgeon fixes the graft inside bone tunnels or at attachment points so it can serve as a new ligament as healing progresses. The graft is living tissue, not an artificial cable, and it changes as it incorporates into the knee. Early protection and structured rehabilitation help the knee and the harvest area recover together.

Where it is used

A hamstring autograft may be selected for reconstruction of a torn ACL when the knee needs a new stabilising ligament. Graft choice depends on age, activity goals, previous operations, other knee injuries, examination findings and the surgeon’s assessment. The graft does not repair every knee problem and does not replace rehabilitation. Exercises progressively address swelling, range of motion, strength, balance and movement control after surgery.

Alternatives to consider

Other reconstruction options include a patellar tendon autograft, a quadriceps tendon autograft or an allograft from donated human tissue. Each has different harvest-site considerations, fixation choices and rehabilitation demands. Some ACL injuries can be managed without reconstruction, depending on knee stability, symptoms and activity needs. A clinician compares these options with you after reviewing the injury and your goals.

What it means for recovery

You may notice discomfort at the front or back of the thigh as well as around the knee after surgery. Follow the weight-bearing, brace, wound-care and exercise instructions provided by your surgical and physiotherapy team. Do not rush pivoting, jumping or sport-specific drills because the knee can feel better before the graft and surrounding tissues are ready for those loads. New calf swelling, chest symptoms, fever, wound drainage or a sudden loss of knee movement needs prompt medical advice.

Graft healing and safety

The reconstructed ligament needs time to attach and mature, while the hamstring harvest site also rebuilds strength. Follow-up appointments allow the team to check motion, stability, strength and progress through rehabilitation. Protect the knee from twisting falls and unplanned impact during the stages when your clinician advises caution. Pain that is escalating, a locked knee, repeated giving way or signs of infection should be assessed rather than managed by changing exercises alone.
