What should you know about zosteriform lichen planus?

Lichen planus is an inflammatory skin condition that may cause itchy, flat-topped or slightly raised bumps with a purple, reddish, or brown tone. In the zosteriform pattern, lesions follow a strip or segment on one side of the body, sometimes along the path of a nerve. The distribution can look like shingles, but the conditions are different and may need different management.

A clinician may inspect the colour, surface, arrangement, duration, pain, itch, and any mouth, scalp, nail, or genital changes. A skin biopsy or other assessment may be considered when the appearance is unusual or the diagnosis is uncertain.

Avoid scratching, fragranced products, and harsh rubbing over the affected area. Keep a record of when the rash began, whether it is spreading, and any new medicines or illnesses around that time. Treatment may include a prescribed topical corticosteroid for local inflammation; more extensive or resistant disease may lead a dermatologist to discuss oral corticosteroid treatment or another option. Use medicines only as directed.

Arrange prompt assessment for a new one-sided band of painful blisters, rapidly spreading lesions, fever, eye involvement, or sores in the mouth or genital area. Seek urgent care for eye pain or vision changes. These features can require time-sensitive evaluation and should not be assumed to be lichen planus from appearance alone.

Ask: Could this pattern be shingles or another rash? Do I need a biopsy? Are my mouth, nails, scalp, or genital skin involved? Which treatment fits the location and thickness of the lesions? How long should I use the prescribed medicine, and what change should trigger review?
