Why does osteomyelitis cause persistent bone pain?

Osteomyelitis is an infection of bone or the tissues around it. The immune response brings swelling and fluid to the infected area. Because bone is enclosed by firm tissue, this pressure can produce a deep ache or tenderness. Nearby muscles and joints may also become stiff, making movement more uncomfortable.

Pain may occur with fever, warmth, redness, swelling, drainage from a wound or reduced movement, but some people have few outward skin changes. Persistent bone pain has many possible causes, so symptoms need examination rather than self-diagnosis.

Seek medical assessment for ongoing, worsening or focal bone pain, especially after surgery, an open injury, a deep wound or an infection nearby. A clinician may examine the area and use blood tests or imaging; a sample may be needed to identify the organism and guide treatment.

Take prescribed antibiotics exactly as directed and protect the painful area as advised. Do not squeeze a draining wound or use leftover antibiotics, because this can delay the correct treatment.

Get urgent help for severe or rapidly increasing pain, fever with shaking chills, confusion, faintness, rapidly spreading redness, new weakness, or pus from a wound. Emergency care is also needed if an infected limb becomes cold, pale or difficult to move.

Ask whether the pain could reflect bone infection, which tests are needed, how long treatment may be required, how to protect movement, and which changes should prompt immediate review.
