Why does Osgood-Schlatter disease cause kneecap pain?

Short answer
Most people with Osgood-Schlatter Disease notice kneecap pain because force travels through the patellar tendon to an irritated growing attachment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

How pain reaches the front of the knee
The kneecap moves within the quadriceps and patellar tendon system as the knee bends and straightens. The tendon attaches just below the kneecap, where repeated pulling can irritate the developing bone. Pain may therefore be felt at the lower edge of the kneecap or across the front of the knee, particularly with stairs, squats, sprinting, or jumping. Kneecap Pain is a common presentation of Osgood-Schlatter Disease, but front-of-knee pain can arise from several conditions.
Practical steps
Notice which activities trigger symptoms and reduce their intensity or frequency for a while. Avoid kneeling directly on the tender area. A cold pack covered with fabric after activity may ease soreness. Keep gentle knee movement, and ask a physiotherapist about progressive thigh and hip exercises that do not sharply increase pain. Return to sport gradually when everyday walking and basic movements are comfortable.
When to seek care
Arrange an assessment when pain is persistent, worsening, or stopping normal activity. Seek prompt help if the knee becomes very swollen, hot, or red, or if fever occurs. After a forceful injury, urgent assessment is appropriate for inability to bear weight, a visible deformity, a locked knee, or sudden severe pain. A clinician can examine the tendon and joint and clarify whether the activity plan needs adjustment.
Questions for your doctor
Where exactly is the irritated attachment? Which sports movements should be modified? What exercises are appropriate now? When is a gradual return to training reasonable? Could another cause explain the pain?

Questions people ask
Why does jumping hurt the kneecap area?
Jumping increases tension in the quadriceps and patellar tendon, which can pull on the sensitive attachment below the kneecap.
Is a tender bump part of the condition?
A firm prominence below the kneecap can occur, but a clinician should assess a new or changing lump.
