Why does myofascial pain syndrome cause upper back pain?

Short answer
Upper Back Pain is a very common presentation of Myofascial Pain Syndrome because overloaded muscles around the shoulder blades can become tender, guarded, and painful with movement.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common location
- Between or beside the shoulder blades and along the upper spine
- Movement focus
- Comfortable shoulder, rib, and upper-spine motion with gradual strengthening
What this means
Muscles that support the shoulder blades, upper spine, and ribs work together during reaching, lifting, breathing, and sustained sitting. A sensitive tight band in one of these muscles may cause a sore spot between or beside the shoulder blades. Pain can also be referred, so the area that feels painful may be wider than the original trigger point.
When the region hurts, the body may hold the shoulders or upper spine stiffly. That guarding can increase fatigue during desk work, phone use, driving, or overhead activity. Upper back pain can also arise from the neck, ribs, joints, or internal problems, so the pattern and examination matter.
What you can do
Break up long periods of sitting with easy walking and relaxed shoulder movement. Gentle thoracic extension, shoulder-blade motion, and breathing exercises may help if they remain comfortable. Heat can be soothing for muscle guarding, but protect the skin and avoid using excessive heat.
A physiotherapist can observe how your neck, ribs, shoulders, and upper spine share movement. They may prescribe progressive strengthening for the shoulder girdle and advice on reaching, lifting, or workstation setup. Increase activity gradually instead of testing the area with sudden heavy effort.
When to seek care
Get an assessment if upper back pain lasts, disrupts sleep, or limits breathing, reaching, or daily tasks. Seek urgent care for chest pressure, shortness of breath, fainting, sudden neurological symptoms, fever with severe pain, or pain after major trauma.
Questions for your doctor
Does my examination support a muscle and fascia source? Could my neck, ribs, or shoulder be contributing? Which movements should I practise, and what changes would require prompt reassessment?

Questions people ask
Can myofascial pain be felt between the shoulder blades?
Yes. Sensitive muscles that control the shoulder blades can create local tenderness or refer aching into that region.
Can posture contribute to upper back symptoms?
Sustained positions can increase muscle loading and guarding, although posture is only one possible contributor and is not a diagnosis by itself.
