Why Does a Distal Radius Fracture Cause Hand Swelling?

A distal radius fracture is an injury near the wrist end of the forearm bone. The fracture and surrounding soft-tissue injury trigger inflammation, which draws fluid into the area. Because the hand hangs below the wrist, fluid can move into the hand and fingers, making rings feel tight and finger movement harder. A cast, splint, or reduced movement can add to stiffness. Bruising may also spread down the hand as blood tracks through the tissues. Swelling should be monitored alongside finger colour, warmth, sensation, and movement.

Follow the fracture team’s instructions about the splint or cast. Unless you were told otherwise, keep the hand elevated above heart level when resting and gently move the fingers that are free to move. Avoid tight rings and check that swelling is not making the support feel constricting. Do not alter or remove a cast yourself. Cold may help early discomfort when used safely around, not directly on, a cast or dressing.

Contact the fracture service promptly if swelling is increasing, the cast feels tight, or pain is not controlled as expected. Seek emergency help if fingers become blue, pale, cold, increasingly numb, very difficult to move, or severely painful despite elevation. New deformity, an open wound, or rapidly worsening pressure also needs urgent assessment because circulation or nerve function may be affected.
