Why does chronic kidney disease cause muscle cramps?

Healthy kidneys help regulate water and minerals such as potassium, calcium and magnesium. When kidney function is reduced, shifts in these substances can affect how nerves signal and how muscle fibres contract and relax. Medicines, reduced appetite, dehydration or removing fluid during dialysis can add to the problem. Muscle Cramps is a common presentation of Chronic Kidney Disease, but cramps also have many other possible causes.

A cramp is often a sudden, tight, painful knot in a calf, foot, thigh or hand. It may leave the muscle sore afterwards. Note when it happens, how long it lasts, what you were doing, and any recent changes to medicines, fluid intake or dialysis treatment.

Gently stretch the affected muscle and move it slowly once the sharp pain eases. Keep to the fluid, diet and medicine plan provided by your kidney team; do not deliberately increase fluids or take mineral supplements without their advice, because kidney disease changes what is safe for you. Bring a symptom record to your next appointment so the team can review blood tests, medicines and dialysis settings if relevant.

Contact your kidney team promptly for new frequent cramps, cramps that disturb sleep, or symptoms that began after a medicine or dialysis change. Seek urgent medical help for cramping with chest pain, severe weakness, fainting, marked breathlessness, confusion, a fast or irregular heartbeat, or inability to move a limb.
