Why does cancer of unknown primary cause neck lump?

Neck Lump is a common presentation of Cancer of Unknown Primary. Lymph nodes in the neck filter fluid from nearby tissues, and cells can reach them through lymphatic channels. A node may enlarge as cancer cells multiply inside it. The primary tumour may be very small, hidden in a difficult-to-see area, or no longer identifiable after the first assessment. A lump can also arise from infection, inflammation, a cyst or another non-cancerous cause, so its presence alone does not establish a diagnosis.

Clinicians consider the lump's position, duration, firmness, movement and associated symptoms. Examination of the mouth, throat, skin and other regions may be combined with imaging and a needle sample. Laboratory review of cells can suggest the tissue type and guide the search for a primary site.

Arrange an assessment for a persistent, enlarging or unexplained neck lump. Bring a list of medicines and note when it appeared, whether it changes, and any fever, sore throat, voice change, swallowing difficulty, ear symptoms or weight change. The clinician may examine the head and neck, request imaging and recommend a biopsy. Avoid repeatedly pressing the lump, which can make it sore and makes changes harder to judge.

Seek urgent medical help if the swelling causes trouble breathing, choking, inability to swallow fluids, rapidly increasing pressure, severe bleeding or marked weakness. Contact the treating team promptly for new voice change, worsening swallowing, coughing blood, or a lump that is enlarging. These symptoms need direct assessment and should not be explained away by the cancer's unknown starting point.
