Whole-body hives: what they mean and what to do

Whole-body hives are raised, itchy welts that can appear across many areas of skin and may come and go as the body releases histamine. Individual welts can change shape, join together, fade in one place and appear elsewhere. The skin may feel warm or prickly, but hives do not always have the same appearance on every skin tone.

Widespread hives can follow an infection, a medicine, an insect sting, a food exposure, heat, pressure, exercise or emotional stress. Sometimes no clear trigger is found. Hives are a skin reaction rather than a diagnosis of the trigger itself. A clinician may ask when the welts started, whether they move, what you recently ate or took, and whether you have swelling of the lips, eyelids, tongue or throat.

Deep swelling beneath the skin is called angioedema and can occur with hives. Scratching may intensify irritation and break the skin, so note the pattern with photographs and avoid suspected triggers until you receive medical advice.

Move to a cool environment, wear loose clothing and use a cool damp cloth on uncomfortable areas. Avoid hot showers, vigorous rubbing and any newly started product that may be irritating the skin. Keep a record of recent medicines, foods, illnesses, stings and activities; this can help a clinician assess possible triggers.

A pharmacist or doctor may recommend an oral antihistamine for itch and welts after checking your age, other medicines, pregnancy or breastfeeding status, and relevant health conditions. Do not combine products or exceed label directions without professional advice. Corticosteroid treatment may be considered for selected severe flares, but it is not suitable for everyone.

Seek emergency help immediately if hives occur with trouble breathing, wheezing, throat tightness, a hoarse voice, difficulty swallowing, faintness, confusion, or swelling of the tongue or mouth. These symptoms can signal a serious allergic reaction, even if the skin welts seem mild.

Arrange prompt medical assessment when the rash is rapidly spreading, the swelling is significant, symptoms follow a new medicine or sting, or the episode keeps returning. A clinician can distinguish hives from other rashes and decide whether testing, treatment or review of a medicine is needed.
