White hard spots on skin

Small, smooth, pearly spots that feel firm may contain trapped keratin. Spots with redness, warmth, tenderness, or a central hair can fit folliculitis or an ingrown hair. Rough bumps on the arms or thighs may have a different pattern from facial acne. New products, shaving, friction, and sweating can irritate follicles and create white-tipped bumps. Acne can include whiteheads, while hormonal changes can influence where breakouts recur. A spot is a symptom, not a diagnosis, and the same colour can appear in unrelated conditions.

Wash with a mild cleanser and avoid scrubbing, picking, and home piercing. Choose non-comedogenic moisturiser and sunscreen if the area is exposed. Stop a newly introduced product if it burns or causes a rash, and change damp clothing after sweating. If shaving triggers spots, use a clean sharp razor, soften the hair first, and avoid repeated passes over the same area. A clinician can distinguish a keratin-filled bump from acne or folliculitis and select a safe removal or prescription option. Do not use strong acids or steroid creams on an undiagnosed spot without guidance.

Book a skin assessment when spots persist, spread, recur in the same area, or do not respond to gentle care. Seek prompt medical attention for rapidly spreading redness, marked pain, pus, fever, facial swelling, or a spot that bleeds or changes. Urgent assessment is also appropriate for a new lesion near the eye with vision symptoms.

Ask what the spots contain, whether shaving or a product is contributing, and whether treatment should wait until irritation settles. Show photographs from when the spots first appeared and list any medicines or skincare used on the area.
