When should you worry about toe pain if you have turf toe?

Short answer
Toe pain is a common presentation of turf toe, and increasing pain, swelling, bruising, or reduced big-toe movement warrants assessment.

What turf toe symptoms can mean
Turf toe is an injury to the soft tissues around the big-toe joint, often after the toe is forced upward while the foot is planted. Pain may be felt beneath or around the joint, with swelling and bruising. Stiffness can make push-off difficult, and the joint may feel weak during walking. Symptoms can vary with the severity of the injury, so a clinical examination helps determine how much protection and rehabilitation are appropriate.
What to do at home
Stop the activity that caused the pain and limit running, jumping, and forceful push-off. Use a firm, supportive shoe that limits bending at the big-toe joint. Elevate the foot when it is swollen and apply a wrapped cold pack for short intervals. Do not repeatedly test the joint by bending it upward. If walking remains painful or the symptoms are not settling, arrange a medical review.
When to seek care
Get prompt care if you cannot bear weight, the big toe appears out of position, swelling or bruising is marked, or movement is suddenly very limited. Numbness, a cold or pale toe, or severe pain after a direct injury needs urgent attention. Assessment may include checking joint stability and deciding whether an X-ray or other imaging is useful.
Questions for your doctor
Ask what grade of injury is suspected, how long to use a stiff-soled shoe, and which movements should be avoided. Ask when strengthening and balance work can safely restart and what symptoms should interrupt rehabilitation.

Questions people ask
Why does turf toe hurt when walking?
The big toe helps the foot push off, so loading or bending the injured joint can reproduce pain.
When should sport stop?
Stop sport when it increases pain, swelling, or difficulty pushing off, and seek advice about a graded return.
