What the symptoms can mean

The plantar plate is a strong structure beneath a toe joint that helps keep the toe stable. A tear can cause pain under the ball of the foot or at the base of a toe, especially when pushing off. Swelling, tenderness, or a feeling that the toe is unstable can occur. A toe that begins to lift, cross, or drift may indicate that the joint is losing support. These findings do not establish the cause by themselves, because several foot problems can produce similar pain.

What you can do now

Reduce activities that load the front of the foot, such as running, jumping, and repeated barefoot walking. Wear a supportive, roomy shoe and avoid footwear that bends the toes sharply. Rest the foot and use a wrapped cold pack for short periods if swelling is present. Do not forcefully stretch or manipulate the painful toe. Arrange an assessment if symptoms continue, interfere with ordinary walking, or return whenever activity increases.

When to seek care

Seek urgent care for a toe that looks suddenly deformed, becomes numb or unusually cold, changes colour, or cannot be moved after an injury. Prompt clinical review is also sensible when pain is severe, swelling is rapidly increasing, or you cannot bear weight. A clinician can examine stability, compare alignment, and decide whether imaging or protection of the joint is needed.
