When to worry about a skin rash with ringworm

Ringworm is a fungal infection, not a worm. It may form an itchy, scaly patch with a more active edge and clearer centre, although appearance varies by body site and skin tone. Scratching can spread fungal material to nearby skin or other people. A rash on the scalp, beard area, or nails can need a different treatment approach from a small patch on the body.

Keep the area clean and dry, wash hands after touching it, and avoid sharing towels, clothing, hair tools, or sports equipment. Use an antifungal product only as directed on its label or by a clinician, and complete the recommended course even if the surface looks better. Avoid steroid-only creams unless a clinician specifically advises them, because they can mask or alter a fungal rash.

Arrange an assessment if the rash reaches the scalp, beard, or nails, affects a child, involves a large area, keeps spreading, or does not improve with appropriate treatment. Seek prompt care for pus, increasing pain, warmth, swelling, fever, or red streaking. Urgent help is needed for facial swelling, breathing difficulty, or rapidly worsening illness.

Ask whether the rash needs testing, which body sites are involved, and how long treatment should continue. Mention contact with affected household members or animals, previous antifungal use, scalp or nail changes, and any immune or medical conditions.
