When to worry about shortness of breath with asthma

Shortness of Breath is a very common presentation of Asthma. During an asthma flare, the airways narrow and become inflamed, making it harder to move air out. Wheezing, cough, chest tightness, and fast breathing may occur with it. Triggers can include viral infections, smoke, dust, exercise, or allergens. Breathlessness that is different from your usual pattern may have another cause and should not automatically be treated as asthma.

Stop the activity, sit upright, and stay as calm as you can. Use your reliever inhaler according to your written asthma action plan, using a spacer if one has been prescribed. Keep track of how well it works and whether symptoms return quickly. If you need the reliever more often than your plan allows, arrange same-day medical advice. Do not borrow an inhaler or change preventer treatment without clinical guidance.

Call emergency services if you are struggling to breathe, cannot speak in full sentences, become drowsy or confused, develop blue or grey lips, or have severe chest tightness. Get immediate help when your reliever is not helping or symptoms worsen after using it. Sit upright while waiting and take your medicines and action plan with you. A sudden breathlessness episode with facial swelling or hives is also an emergency.

Ask whether your inhaler technique is effective, when to use a spacer, and what your personal action plan says during a flare. Discuss night symptoms, activity limits, trigger avoidance, and whether your preventer needs review.
